When a Baby's Head Preference Is Just the Tip of the Iceberg

When most parents hear the word torticollis, they think of a baby with a head tilt or a preference for looking to one side.

But in practice, that's often just one piece of a much bigger picture.

As a pediatric chiropractor serving Miami families, I've found that many babies with torticollis are also struggling with feeding challenges, tummy time, sleep, reflux, congestion, and overall comfort in their little bodies.

That was exactly the case for Romi.

"Something Just Didn't Seem Comfortable"

When I first met Romi, her parents were concerned about her strong head preference. She consistently looked to one side and didn't seem comfortable turning the other direction.

But there was more going on than just her neck.

Romi hated lying on her back.

Tummy time was a daily struggle.

During feedings, milk would dribble out the sides of her mouth, and she seemed to work incredibly hard just to eat comfortably.

When her parents held her, she felt stiff and tense.

She spit up frequently and dealt with daily congestion.

Most importantly, she didn't seem fully at ease.

As parents, we often know when our babies are uncomfortable, even if we can't always explain why.

Looking Beyond the Neck

Infants with torticollis commonly experience symptoms that extend far beyond a visible head tilt, including difficulty feeding, reflux, irritability, sleep challenges, and a strong preference for positioning on one side.

While the neck muscles are certainly involved, many providers are beginning to recognize that torticollis often affects the entire baby's movement patterns and comfort level.

A baby who doesn't feel comfortable turning their head may also struggle to:

  • Feed efficiently on both sides

  • Enjoy tummy time

  • Rest comfortably on their back

  • Sleep peacefully

  • Explore movement symmetrically

  • Reach developmental milestones with ease

This is why I never look at torticollis as simply a neck problem.

I look at the whole baby.

And perhaps most noticeably, she is happy.

She is smiley, relaxed, and easygoing in a way that her parents weren't seeing before.

The neck preference that first brought her into the office was important—but it wasn't the whole story.

When her body became more comfortable, everything else began to change too.
